Caregiver Application Requirements for Patients Over 18

Caregiver Age Requirement

 A caregiver is a person who, once registered with the Maryland Medical Cannabis Commission and in possession of a valid MMCC Caregiver ID card, is able to purchase medical cannabis from a licensed Maryland dispensary on behalf of his or her designated patient(s) and transport the legally obtained medical cannabis to the patient(s). Once registered, a caregiver may serve a maximum of five registered patients at one time.

You will need an electronic copy of a clear photograph taken within the past 

6 months similar to a driver's license or passport photo featured on plain white or off-white background. The MMCC recommends using a  highly-rated passport photo app downloaded from Apple or Google Play Store.  


Your face must be unobscured in the photo and you must be clothed, directly facing the camera with both eyes open. Avoid wearing a hat, head covering, sunglasses or dark glasses. In the photo, you should have a neutral facial expression or natural smile.

Documentation Registration Requirement

There are certain documents you will need to accompany your registration if the identification you provide is not a Maryland driver’s license or MVA ID Card or does not identify your current address.  You must also provide two of the following documents as proof of your current Maryland address:  

  • MVA registration card or title.
  • Utility, telephone or cable/satellite TV bill.
  • Bank account statement.
  • Property tax bill.
  • Mortgage statement or proof of home ownership (Deed, Title, Bill of Sale).
  • Major cred card bill (Visa, MasterCard, Wal-Mart, Target, etc.).
  • Residential rental  contract (lease or rental contract between landlord and tenant which ​includes all signatures).
  • Selective Service Card.
  • Federal, state or local government mail issued by the state of Maryland (e.g. Social Security or Social Services).
  • Car insurance policy.


Documents must include the applicant's name and address and may not be from the same two agencies, e.g. two documents issues by the MVA.  All documents must be dated within the past 90 days.

Caregiver registration is completed entirely online. The online application form must be completed and submitted in one session; you will not be able to save and complete the application at a later time. It is essential that you have the required information and documents in an electronic format ready for upload before starting the application process. 


Carefully review the requirements provided as well as documents needed to ensure you are prepared to complete the entire application before beginning the process.


Steps to register as a caregiver:

  1. Complete and submit an online application on Maryland One Stop at  Go to "Browse by State Agencies" and go to Maryland Medical Cannabis Commission and choose "Caregiver Registration."
  2. Gather all of the necessary photo and documentation requirements to complete the application.  
  3. Once application is complete, verify your email address following application completion.
  4. Once your application has been approved,  the patient must log into their account and designate you as a caregiver in the One Stop Portal.
  5. Purchase ID Card once registered and have received MMCC approval. 
  6. Once designated as a caregiver, you may visit a licensed Maryland Dispensary for the patient.

Caregiver Registration Requirement

 Minor patients (under the age of 18) are required to have at least one caregiver at all times, who is a parent or guardian of the minor. A minor patient may have up to four caregivers, two of whom are a parent/guardian, and two additional adults over the age of 21 designated by a parent or guardian. A minor patient’s caregiver must register with the Commission prior to registering the minor patient.

You will need to demonstrate proof of guardianship and complete the Minor Form.

  1. An electronic copy of official proof of the caregiver’s guardianship of the minor patient.  Acceptable documents include the patient’s birth certificate, adoption certificate or a court order awarding guardianship. 
  2. An electronic copy of the completed and notarized Minor Patient Form.   Please download and complete the Minor Patient Form in its entirety, have it notarized, then scan or take a clear picture of the completed form.

Steps to register a minor patient:

  1. Register the minor patient’s caregiver.   Minor patients (under the age of 18) are required to have at least one caregiver at all times, who is a parent or guardian of the minor.  The caregiver must submit an online caregiver application and be approved by MMCC before registering the minor patient. Once fully registered, the caregiver will be issued an ID Number by MMCC, which is needed to complete the minor patient application form.
  2.  After the minor patient’s caregiver registers, the minor patient’s registration can commence. 
  3. ​​​​​​​​​​​​Gather the required files and information. This includes the information outlined above under Identification Requirements and Documentation Requirements of the Caregiver, Photo Requirements of the Minor, and the Proof of Guardianship as well as Minor Patient Form.
  4. Complete minor patient's application.
  5. To complete the minor's application, you must have the MMCC Caregiver's number.
  6. Verify caregiver's email address.
  7.  Once the minor patient’s application has been approved, the patient is ready to visit a provider registered with MMCC to obtain a valid certification. 
  8. Once the  minor is certified by a provider, the caregiver can visit a licensed Maryland dispensary to purchase medical cannabis for the minor patient.
