ChARM PHR is a Personal Health Record (PHR) portal to manage health information of patients, for themselves and their families. Patients can store their current and past medications, lab results, record health vitals and track progress using the built-in reports. All the health information is available online, thus avoiding rummaging of files to find past lab results.
Patient Portal enables patients to share lab records with physicians and also allows patients to view the documents shared by their physicians. Patients can request appointments with their care team, which gets securely notified to the physician’s ChARM EHR account. When the physician confirms the appointments, and shares pre-appointment questionnaires, patients can fill them up from their PHR account, so that the physician can know about the patient’s health information better, before the patient checks-in into the practice.
You can register a patient portal account by accepting the invitation email sent by your practice.
If you have not received the invitation email:
Once you have the invitation email, follow the steps below to register your account.
Step 1:
DOB verification is done to confirm that you are authorized to access this patient’s medical records.

Note: If the username that you have entered is already in use, you will be asked to enter a different name.
Please remember the username and password. In case, if you forgot the password, use the ‘Forgot Password’ link on https://charmphr.com/ to reset your password.
Once the account registration completes, you will be automatically redirected to patient portal and also you will get a Welcome Email.
In addition, you can also login to patient portal from ChARM PHR home page at https://charmphr.com
Note: ChARM PHR employs strict privacy rules. No one can look at the data except you or those designated by you.
You can fill pre-appointment questionnaires shared by your practice prior to your office visit. This significantly reduces the time you spend in waiting room during appointment and helps your provider to know about you in advance. When your practice shares such questionnaires with you, you will get an email notification. In addition, the Timeline view will show you the notification, and the main menu will get updated with count in bracket.
You can fill pre-appointment questionnaires from 'Timeline' or 'Appointments' or 'Questionnaires' section.
Choose a questionnaire, fill the details and click on 'Save' for to save it as draft. Once you complete filling the questionnaire, you can click 'Submit' to send the document to the practice.
The ‘Current’ tab lists the questionnaires to be filled for your upcoming appointment. The ‘Past’ tab lists the questionnaires filled for previous consultations.
Note:
Create a Folder
You can create your own folder using the '+' icon present next to 'Folders'. Provide a 'Folder Name' and 'Folder Description' to create the folder.
Upload Documents
You can upload your documents using Upload button present in the 'Documents' section. While uploading, you can select the folder, to save the document to that folder.
Share Documents
You can share your documents to your practice and to others(non practice members) from the following places
Quick “Share” menu is already explained. Let us see the share option from “Documents” section.
Share Option in Documents
You can share your documents from either 'Bulk Action' bar or 'Row Action' bar.
Bulk Action Bar - Contains links to perform group actions like share, delete, move, etc. This can be view by selecting the check box present against each of the row.
Row actions - Contains links for actions that can be performed on that row.
Share Documents with Practice
When you select “Single” or “Bulk” share option as shown above, the following dialog will be shown

When you click the 'Send Message' or 'Compose' the compose dialog will be shown as below.
Steps:
Message will be delivered to the respective practice member and copy of the Sent message will be available in “Sent” folder.
